Create forms riders fill in online, such as extra waivers, contracts, surveys, waiting lists and competition entries. Riders fill in a form on your Hopoti page, from a link you share, or when they book. The form every rider signs before booking has its own article: Set up the Registration Form riders must sign.
This feature is available on certain plans
Online waivers and forms are available on the Pro and Ultimate plans.
Earlier plans (from before September 2026) include this too.
Create a form
Select Forms in the admin menu.
Open the Edit forms tab.
Select Add new form.
Choose the form type: Other waivers, Contracts, Surveys, Waiting lists, Competitions or Forms for anything else.
Add fields from the list on the right and drag them into order.
Select Preview to see the form as riders see it.
Set Published to Yes and select Save.
Riders can now fill in the form. Until you publish it, the form is a draft only your stable sees. Notes at the top are for your staff and aren't shown to riders.
You choose which fields riders must fill in. Besides Header, Paragraph, choices (Select, Checkbox Group, Radio Group), Text Field, Text Area, Number and 1-5 Star rating or Emoji rating, there are three special fields:
Signature: a signature with a name clarification field.
User information: the rider's profile details when they send the form.
Horse selection: one horse from your public list of horses.
Choose where the form shows and who gets copies
Open the form on the Edit forms tab, select Form settings, make your choices and select Save:
Show on front page: Yes lists the form on your Hopoti page. With No, riders reach it only by link or when they book.
Fill with every booking: for forms on lessons or services. No skips riders who have filled the form before, which suits waivers and contracts. Yes asks on every booking.
Create PDF, Send filled form to client email and Send filled form to business email: a PDF copy of each filled form for the rider, your stable or both. Your copy goes to the Forms email address in Settings > Settings > Business emails.
Form category settings: a starting category for every answer, to help you follow them up.
PDFs and email copies make booking a little slower, so switch them on only when you need a copy. Filled forms are always saved in Hopoti: see Review and export form answers.
Ask riders to fill in a form when they book
Add a form to a lesson when you need answers before the lesson, such as a horse wish or dietary needs.
Open the lesson under Events and open the Edit tab. For a series, select the first lesson that should get the form.
Under Event forms, choose one or more forms.
Select Save changes to this event for this lesson only, or Save changes to all events > Only save changes to all events for this and every later lesson.
Riders are now asked to fill in the form when they book the lesson, and their answers show in the lesson window next to each rider.
For a service, you add the form to each service option instead: open the service, open the option riders book and choose the form in the option's settings. Riders then fill it in when they book that option.
A Membership agreement works the same way: add it as a form on the weekly lesson, and riders sign it when they join the Membership. See Set up a monthly Membership for lessons.
Share a form by link
On the Edit forms tab, select Share next to the form to get a link riders can open to fill it in, for example in an email, a feed post or on social media. A crossed-out eye next to Share means the form isn't on your Hopoti page, so riders can only reach it by link or when they book.
Change a published form
Editing a published form creates a new version of it. Answers to earlier versions are kept, and the form's summary shows each version separately because the questions may differ. To start a similar form instead, open the form and select Duplicate.
